A 56 year old woman before and after upper blepharoplasty, upper ptosis repair, and lower blepharoplasty with fat transfer over the orbital rim and elevation of the muscles of the midface.
Comment: No fat was removed from the upper eyelids to avoid the appearance of aging. The bulge in the lower eyelids are smoothed while the line between the lid and the cheek is softened.

Note: The prominent grove between the skin of the nose and the lower eyelid has been reduced. Instead of being removed, lower eyelid fat is placed to fill the groove.

Comment: While ptosis repair is often performed to improve impaired vision, many patients may benefit cosmetically by this procedure. Simple removal of eyelid skin and muscle without PTOSIS repair would NOT have made this patient look more awake and younger.
